What form do I fill out to request an "Order to Show Cause" for Child Support in Utah?

My ex is over 14K behind and everytime ORS finds him he quits, I am tired of tracking him down and think it's time he see's a judge to explin himself. Does anybody know how I can start this process on my own, I cannot afford a lawyer?
